Insurance Options That Work For You
When you receive care at VitalStrong Outpatient Rehab, we will bill your health insurance for you. To do this, we will ask for a copy of your insurance card(s). It is important that you provide us with complete and accurate information.
We accept many major insurance plans and private payments. If your plan requires authorization or referrals, we will help you get those. If you have questions about your individual coverage, please confirm with your insurance plan.
VitalStrong is able to bill most common insurance plans as a secondary payment and can also provide treatment as an out of network provider for multiple plans.
We are currently in-network and a preferred provider with:

Payment Detail – Copays are required at the time services are provided. Payment is accepted via credit card, debit card, check, money order or cash app.
Please note that insurance providers are subject to change. We strive to update our website as expeditiously as possible and will confirm your insurance upon check-in at VitalStrong Outpatient Rehabilitation.

In advance of your visit to a VitalStrong Rehabilitation Location, please complete the Universal Patient Intake Form and the relevant location Consent To Treat Document.
